Forms are created in Administration Portal. To learn more about creating forms, please visit Form Builder documentation. Once a Form has been created it is available to user to complete in both the app and the portal and completed Form Responses can be viewed in the Form Responses subtab.
Access Form Entry subtab from the Data Entry tab in the Students Section.
- All available Forms will display.
- Click on desired Form for entry.
- Enter fields for Form.
Note: Required and available fields will vary between Forms depending on how they were built.
- You can now sign forms electronically in the Portal. To do so, type you name and select the checkbox labeled I acknowledge this is the digital equivalent of my signature.
- Click Save & Close when finished.
